Requesting Records (DACE)
Before you start
- Requester info: first/last name, email (phone optional).
- Student info: first/last name, date of birth, and last LAUSD Adult school attended (searchable list).
- Record type you need (see Fees).
- Delivery: Electronic (PDF) or Mail (USPS).
- Payment: Credit/Debit (online) or Money Order.
- Some requests include additional custom fields that may be required.
Step 1 — Requester Information
- Enter your name and email (phone optional).
- Optionally enable SMS updates for request status.
- If you’re a third party, check “This is a third-party request” and be ready to upload authorization.
Step 2 — Student Information
- Enter first/last name and select Date of Birth (past dates only).
- Choose the Last School Attended in LAUSD (use Adult school entries in the combobox).
Step 3 — Request Details
- Pick a Record Type. The total fee appears next to each option.
- If custom fields appear, complete required items before continuing.
Step 4 — Delivery & Payment
Delivery
- Electronic (PDF) — receive a secure download link via email/SMS.
- Mail (USPS) — add one or more Mailing Addresses:
- Use Search / Full address to validate (status shows Validated), or enter manually and then validate.
- Set Number of Copies per address. Fees scale with total copies.
Payment
- Credit/Debit — pay online; your request submits immediately.
- Money Order — after submit you’ll get the amount and mailing instructions; processing starts after payment is received.
Submit & track
Click Submit Request. On success you’ll see a confirmation with your tracking number and receive email/SMS updates.
