Requesting Records (SPED)
Before you start
- Requester info: first/last name, email.
- Student info: first/middle/last name, date of birth (past dates only; 1900-01-01 minimum), and last LAUSD school attended (use the searchable list).
- Record types: select one or more — IEP, Psychological Report, Assessments.
- Recipients: at least one mailing address (validated) and optional response email.
- Authorization: required to submit.
- Uploads (optional): supporting docs (PNG, JPG, PDF, DOCX; up to 10MB).

Step 1 — Requester Information
Enter your name and email.
Step 2 — Student Information
- Enter student names.
- Select Date of Birth using the calendar (future dates are blocked; earliest allowed is 1900-01-01). :contentReference[oaicite:2]
- Pick Last School Attended in LAUSD from the combobox.
Step 3 — Records Requested
- Provide a time period (optional), e.g.,
2018–2022. - Check all Record Types you need: IEP, Psychological Report, Assessments.

Step 4 — Send To (Recipients)
- Add Mailing Address(es). For each address:
- Set Number of Copies (min 1).
- Use Search / Full address to populate fields and mark as Validated, or enter manually and then validate.

- Optionally add a Response Email and Telephone.
Step 5 — Authorization & Uploads
- Authorization: check the consent box to release records (required).

Submit & track
